A medication reconciliation is important because slowed gastric emptying can alter the absorption of some oral drugs, and patients on insulin or sulfonylureas may need those agents adjusted to reduce hypoglycemia risk.
Documenting the indication, the baseline values, and the rationale for the chosen agent establishes the reference point against which later response and adverse effects are judged. For patients arriving already using a self-sourced product, the baseline also captures what they report using and at what dose.
Ongoing Monitoring
Monitoring during therapy tracks both efficacy and safety. Efficacy monitoring follows the indication—weight trajectory, glycemic control, or other target measures—at intervals frequent enough to guide titration decisions. Red Flags With Self-Sourced and Research Compounds
A growing number of patients arrive already using compounds obtained from research-chemical vendors, compounding sources, or online marketplaces. These products are frequently labeled for research use only and are not manufactured under the quality controls that apply to approved drugs, so identity, concentration, purity, and sterility cannot be assumed.
Concrete red flags include products with no verifiable labeling, concentrations that do not match the container, reconstitution done under non-sterile conditions, and reuse or sharing of vials and needles. Injection-site reactions, unexpected systemic symptoms, or effects out of proportion to the stated dose may all point to a mislabeled or contaminated product rather than a pharmacologic effect.
The clinical priority is patient safety over judgment. Eliciting an accurate history of what a patient is actually using—without stigma—allows the clinician to counsel on risk, screen for adverse effects, and document the situation honestly. Many of these compounds are investigational, and their unapproved status should be stated plainly.
Documentation and Counseling Boundaries
Documentation should reflect the indication, the informed-consent discussion, the off-label or investigational nature of the product where applicable, and the counseling provided. Recording what the patient reports using, the advice given, and the patient's understanding protects both the patient and the clinician and creates continuity for future visits.
There is an important boundary between describing published labels and common practice and directing an individual toward a specific unapproved regimen. Explaining what a label states, what a class does, and what risks a practice carries is education; issuing a personalized dosing directive for a compounded or research product moves into a different domain of clinical and regulatory responsibility.
Counseling is most defensible when it is candid about the limits of the evidence, neutral about practices the clinician did not initiate, and clear about red flags that should prompt the patient to seek care. When a patient's chosen course carries risk the clinician cannot mitigate, documenting the discussion and the recommendation is the appropriate response.
