Why Titration Exists

GLP-1 receptor agonists slow gastric emptying and act centrally to reduce appetite. The severity of these effects correlates strongly with how quickly the dose is raised rather than with the absolute dose reached at steady state.

Titration is therefore a tolerability strategy, not an efficacy strategy. Starting at a low, sub-therapeutic dose and escalating over weeks allows receptor-mediated gastrointestinal responses to attenuate, a phenomenon often described as tachyphylaxis to the nausea signal. Patients who reach maintenance gradually tend to tolerate it far better than those started near the top of the ladder.

Because most agents in this class have long half-lives and reach steady state over several weeks, each titration step is typically held long enough for concentrations to plateau before the next increase. This is the reasoning behind the roughly four-week dwell time built into most labeled schedules.

The Semaglutide Ladder

The injectable semaglutide labels describe a stepwise weekly schedule that begins well below the therapeutic range. For the diabetes indication the labeled sequence starts at 0.25 mg once weekly for four weeks, then 0.5 mg, with subsequent increases toward maintenance doses that the product labeling caps at 2.0 mg for the diabetes formulation. The weight-management formulation follows a parallel ladder that escalates to a 2.4 mg maintenance dose.

As with semaglutide, the 2.5 mg starting dose is a run-in step rather than a target. The labeling frames additional increases as optional and response-driven, meaning many patients maintain benefit at intermediate doses without escalating to the maximum. Published practice descriptions note that continuing at the lowest dose that sustains response is a common pattern.

The dual-agonist mechanism does not fundamentally change the titration logic: gastrointestinal tolerability still governs the pace of escalation, and the four-week interval reflects the same steady-state considerations that apply to semaglutide.

Managing Gastrointestinal Adverse Effects

The most useful lever for GI tolerability is the pace of escalation itself. Dose Conversion: Pens Versus Compounded Vials

Labeled pens deliver a fixed dose per click or per injection, so patients think in milligrams per dose. Compounded multi-dose vials instead present a concentration—milligrams per milliliter—and require the user to draw a volume with a syringe. Converting between the two is the single most error-prone step patients encounter, because a milligram target must be translated into a volume on an insulin syringe.

The arithmetic is straightforward: volume to draw equals the desired dose in milligrams divided by the vial concentration in milligrams per milliliter. The result is then read against the unit markings on an insulin syringe, where 100 units equals 1 mL. A concentration mislabel or a misread syringe can produce a tenfold error, so independent verification of the concentration and the syringe reading is essential.

Compounded GLP-1 products are not FDA-approved formulations and their concentration, purity, and labeling are not guaranteed to match a commercial pen. Clinicians asked to help interpret a vial should treat the stated concentration as unverified and encourage patients to confirm the product and math before drawing any dose. Volume-based conversion tools can reduce arithmetic error but cannot validate the product itself.

Microdosing As A Community Practice

Microdosing—deliberately using doses below the labeled starting step, or escalating in smaller increments than the label describes—is a practice clinicians will increasingly be asked about. In patient communities it is framed as a way to minimize GI effects, reduce cost, or maintain a low steady dose without pursuing maximal weight loss. It is not an approved dosing strategy and its long-term efficacy and safety have not been established in that form.

The rationale patients cite is not implausible: because titration exists to manage tolerability, slower or lower dosing may reduce adverse effects. But sub-therapeutic dosing may also mean sub-therapeutic benefit, and much of the practice relies on compounded vials whose accuracy cannot be assumed. The gap between community practice and labeled use is one of evidence, not necessarily mechanism.

The appropriate clinical posture is to understand the practice well enough to counsel accurately, to be candid about the investigational and off-label nature of what is being described, and to document that counseling. Describing what a label says and what common practice looks like is distinct from directing a specific unapproved regimen.
